You will need to select the type of form you desire to enter your order. If you select table list (most used) a list of the items will be the result (no pictures). Selecting picture catalog will allows the user to enter quantities with the pictures shown for reference.
Usage date range is preselected for 3 months. You may change usage date so your results are based on that usage period. The tesxt list or pictures that will be displayed to place an order are based on the selected deparment (facilitates the overall ordering process). If more than 1 page is needed to display the list or pictures of items, then continuation page numbers will be displayed at the bottom of the form as shown in the figure below.

This screen shows a sample "order entry form" (list form in figure shown). It will list all items purchased by customer in the period specified. The user enters quantities per item, based on what the needs are. Reference is provided if user wants to copy past usage history to make an order (Qty used in period/Mo. Usage)

When you add items to order you will see that this same screen will display "Items Saved" in yellow as shown below:

Additionally you may want to display the items you have already saved by clicking on the "review save items" text link. This will open a window displaying items placed in the order so far.

Once you are ready to checkout, click the "checkout" text menu. User will be presented with a list of all items ordered and allowed to make changes or zero quantities out. Finally when you are confortable with the order you can proceed to Confirm the items. This is a step prior to sending/submitting an order.

You are done, take a moment to review your order and print this for your records and don't forget to send the order with the button at the bottom of the page! When sending the order you will a confirmation. It is recommended that you print this page to have a copy of what you ordered. This page does not allow editing the order but you can always go back to "checkout" if you need to change something!
